The Health Care Financing Administration (HCFA) was set up in 1977 under the Department of Health, Education and Welfare in order to administer the Medicare and Medicaid programs. Later, in 2001, HCFA was renamed the Centers for Medicare & Medicaid Services (CMS).

CMS-1500

The most up to date medical claim form used by physicians and non-institutional providers for claim submission is the CMS1500 form.  The new health claim form contains the fields for reporting the National Provider Identifier (NPI).  The approved collection number for this revised CMS form is OMB-0938-0999 and the revision date is 08-05. 

Before the new 08-05 version of the CMS 1500 form was authorized for use, the 12-90 version was the latest style medical health claim form. Although the old 12-90 version was also called CMS1500, many people referred to it as the HCFA1500.  The approved collection number for these old health forms is OMB-0938-0008.  Prior to the 12-90 version, there was also a 1984 version of HCFA 1500 printed in black.  Both the 08-05 and 12-90 versions are printed in OCR red ink.

UB-04 Billing Form

The UB04 replaced the UB92 form because HIPAA requires that the National Provider Identifier (NPI) also be submitted on the UB forms.  UB-04 forms are the health forms that are used by an institutional provider for billing purposes.  The UB 92 and UB 04 are both under the CMS-1450 title.
